Thank you for choosing St. Lawrence as your choice for high school education. This form should be completed by the parent or legal guardian of the applicant.
You will need the following in a digital format such as a photo for your student:
- Most recent report card
- Recent photo
- Copy of the Baptismal Certificate
- Information page of students passport (International Student’s only)
